Soon, our church will publish the new church member's directory. Please download the form below and submit it using one of the following three ways. Your cooperation will be greatly appreciated. Thank you.

주소 신고 양식(PDF) 다운 받기

다음중 한방법을 택해서 제출해 주십시오.

• PDF 폼을 다운 받습니다. 컴퓨터에서 직접 내용을 기입하신후 파일을 저장합니다. sjkcm [at] sbcglobal [dot] net 으로 이파일을 첨부하여 이메일을 보냅니다. 이메일이 접수되면 2~3 business day 안에 접수확인 답장을 보내드립니다. 만약 4 business day 이후에도 연락이 없으면 이메일이 분실된 것이니 다시 보내주시기 바랍니다.
  

  Download the PDF form. Fill in the form on the computer and save it. Mail to sjkcm [at] sbcglobal [dot] net with the saved file attached. You will get the confirmation mail in 2~3 business days. If you don't get confirmation mail within 4 days, the mail might have been lost. In that case, please send it again.

• PDF 폼을 다운 받습니다. 컴퓨터에서 직접 내용을 기입하신후 프린터로 출력하시든지 아니면 파일을 프린터로 먼저 출력한후 펜으로 내용을 기입합니다. 기입된 양식을 성당 사무실에 제출합니다.

• 성당 사무실에 비치된 양식을 받아서 내용을 기입하신후 다시 사무실에 제출합니다.
